Service Description
A Psychiatric Service Dog (PSD) Assessment is a comprehensive mental health evaluation performed by a licensed mental health professional to determine whether a person meets the clinical and functional criteria for a psychiatric service dog. A thorough PSD evaluation typically includes: * Clinical interview covering mental health history, current symptoms, treatment history, medications, and daily functioning. * Diagnostic assessment to determine whether the individual has a qualifying mental health condition (using DSM-5-TR criteria, when appropriate). * Functional impairment assessment to evaluate how the condition substantially affects one or more major life activities. * Assessment of the need for a PSD, focusing on whether specific trained tasks performed by a service dog could help mitigate the person’s disability. * Review of treatment history and other supports already in place. * Risk assessment, when clinically indicated. * Documentation of the clinical findings and rationale.


Cancellation Policy If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
